- CSC Online Application Form
- Highest Degree Certificate (Notarized copy) If you are a university student then provide Hope Certificate
- Transcripts of Highest Education (Notarized copy)
- A Study or Research plan (not less then 800 words)
- Two Recommendation Letters
- Passport Copy
- Physical Examination Form Photocopy with blood test report
- English Proficiency Certificate (Ielts is not Mandatory)
- 6 Passport pictures (3cm*4cm) with White Background
- Acceptance letter (mandatory ) can’t apply without acceptance letter
