1. CSC Online Application Form (Agency no for B category 11075)
2. Online University form CTGU
3. Highest Degree Certificate (Notarized copy)
4. Transcripts of Highest Education (Notarized copy)
5. Study Plan or Research Proposal (Not Less then 1500 words)
6. Two Recommendation Letters By Professors and Associate Professors (Not from Assistant professors and Lecturer)
7. Passport Copy
8. Physical Examination Form Photocopy
9. English Proficiency Certificate (Ielts is not Mandatory)
10. No Criminal record (police clearance certificate)
