1. CSC Online Application Form (agency number is 10300 For B Category )
2. online application form of  NUIST university
3. Copies Highest Degree Certificate (Notarized copy) if you are in a student in university then provide hope certificate
4. Copies Transcripts of Highest Education (Notarized copy)
5. A Study Plan or Research Proposal
6. Two Recommendation Letters
7. Other supporting documents (For Example published papers, awards & certificates)
8.  Photocopy of applicant’s passport information page
9. Physical Examination Form Photocopy
10. English Proficiency Certificate (Ielts is not  Mandatory)
11. Certificate of No-criminal records
